Abstract Human immunodeficiency virus (HIV) infection and acquired immunodeficiency syndrome (AIDS) are rare in elder patients and the diagnosis could be difficult. We report a case of elder female with pneumo...cystis pneumonia (PCP) associated with AIDS. An 80-year-old female without a history of HIV infection had fever and cough for a week and was treated as bacterial pneumonia by a family physician. Her dyspnea and hypoxia progressed despite of the treatment, and she was admitted to our hospital. Initial CT at the previous hospital showed diffuse ground glass attenuation in the inner areas of the bilateral lung and thin wall cysts in the bilateral upper lung lobe. CT at our hospital showed diffuse consolidation with ground glass attenuation in the inner areas of the bilateral lung. Because CT showed an organizing pneumonia pattern, drug-induced lung diseases or collagen vascular diseases-associated interstitial lung disease were suspected at first. β-D-glucan was elevated and PCP was suspected. She was confirmed HIV infection and diagnosed as PCP associated with AIDS.show more
