| 概要 |
Various ethical issues have arisen in the medical field and the awareness and response to them are thought to affect the clients’quality of life．There is a possibility that midwives，who are involved i...n assisting child delivery，are faced with medical issues and caught in dilemmas of demands between doctors and clients．In this research，therefore，a questionnaire study was conducted with midwives in a certain prefecture．The questionnaire items were taken from the 17 items developed by Okaya et al，asking about ethical awareness and response to issues as well as about the issues midwives were faced with．The 128 valid responses were used for the analysis．As a result，we have found that 1）the midwives’awareness seemed to be affected by their age and career；2）more than 70％of the midwives dealt with the issues by consulting with doctors，clients and a third party；3）some midwives followed directives of doctors which they did not think the best，implying the relationship to the midwives’expertise and independence；4）the issues they were faced with included abortion，pre-natal diagnosis，and handling of personal information；5）almost 30% of the midwives had received no educational training in ethics．Thus the midwives were aware of conflicts in professional values and of the lack in their own self-competency．Their awareness and response to ethical issues seem to be related to their career．Therefore，the necessity of educational training on ethics has been pointed out．
医療の場では様々な倫理的問題を含む状況が生じているが，その問題の認識と対処は医療の質（Quality of Medical Care）の質に影響すると考えられている。医療職の中でも助産師は医療行為である分娩介助を業務としているため，医師と対象者の間の倫理的問題に直面し，ジレンマを感じている可能性がある。そこで，A県下の助産師を対象に，岡谷らが作成した17項目の倫理的問題に対する認識，問題状況への対応および実際に感じている問題に関して質問紙による調査を行い，回答が得られた128名を分析対象とした。その結果，①助産師の倫理的問題に対する認識には，年代や職業キャリアが影響している可能性がある ②倫理的問題への対処として，70％以上の助産師が第三者や当事者との話合いを行っていた ③倫理問題の特性として「最善でないと感じる医師の指示に従う」があり，助産師の専門性や主体性が関与している可能性がある ④実際に感じている倫理的問題は，妊娠中絶，出生前診断，個人情報の取扱いなどがあった ⑤倫理に関しての教育体験がないと回答した者が30％おり，改善の余地がある。以上のことから，助産師は職業的価値観と自己の能力や価値観に関連する倫理的問題を感じており，その倫理的な認識や対処にはキャリアが関連している可能性があることから，医療の場でのカンファレンスや倫理に関する学習の必要性が示唆された。続きを見る
