The Relationships of Sex Hormones, Body Fat Distribution, and Physical Fitness to Lipid and Glucose Metabolism in Men

Abstract The contribution of age, obesity, physical fitness as well as smoking and alcohol consumption in determining the plasma sex hormones(free testosterone : free T, estradiol : E2, dehydroepiandrosterone-...sulfate; DHEA- S) , and sex hormone binding globulin (SHBG) levels, and the relationships of the sex hormones and SHBG to lipid and glucose metabolism were investigated in 217 healthy men aged from 18 to 59 years. The participants in this study included both lean and obese subjects. In a simple correlation analysis, free T was found to correlate significantly with age, body mass index(BMI) waist to hip ratio(WHR), estimated maximal oxygen uptake(VO2max)and alcohol consumption. The level of E2 was significantly correlated with age. WHR as well as alcohol and smoking consumption. The level of DHEA-S significantly correlated with age, while the level of SHBG. significantly correlated with age, BMI, percent of body fat(%fat)and smoking consumption. By means of a multiple linear regression analysis, it was demonstrated that free T was independently and negatively related to age, E2 was independently and negatively related to age and smoking, while SHBG was independently and positively related to age and negatively related to BMI. A simple correlation analysis showed that free T showed a significant negative association with total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), atherogenic index (AI) , triglyceride(TG) , and fasting blood glucose(FBS), while E2 was significantly associated with TC, LDL-C, AI and FBS. The level of DHEA-S showed a significant negative association with AI, TG and FBS, and a positive association with fasting insulin(FIRI). The level of SHBG was significantly associated with TC, high density lipoprotein cholesterol(HDL-C) and LDL-C, and negatively with FIRI. A multiple linear regression analysis indicated that DHEA-S was one of the independent determinants of TG, HDL C and AI, while SHBC. was one of the determinants of HDL- C and FIRI. Furthermore, the E2 level was one of the independent determinants of LDL-C and FBS. The free T was not selected as an independent variable for any dependent variables. This is the first report in which both SHBG and DHEA-S were independently observed to be positively related to HDL C, and thus E2 is considered to be one of the independent predictors of LDL-C and FBS.show more
